Type 2 Diabetes (T2D) remains a significant global health challenge. Several factors may be influencing this increase, including increasing rates of obesity, a sedentary lifestyle, smoking, unhealthy dietary behaviors and genetic factors. As T2D worsens over time, it can
lead to various complications, such as vision problems, nerve damage, kidney problems, heart disease, amputation, and difficulties with
thinking and memory.

A paper was recently published highlighting Healthy Chesapeake’s HUB program, a joint diabetes management program. In addition to providing no-cost medical management to low-income individuals with uncontrolled diabetes and hypertension, the HUB addresses the medical, transportation, and social needs of referred patients.
